BMI Limitations: Why It's Not a Perfect Health Metric

BMI is widely used because it is simple, free, and correlates with health outcomes at the population level. However, it has significant limitations that make it an imperfect measure for individuals.

Does not distinguish muscle from fat: Muscle is denser than fat. A muscular athlete at 95 kg and 180 cm has a BMI of 29.3 — "overweight" — despite having very low body fat. BMI alone would misclassify this person. Body fat percentage, waist circumference, and other measurements tell a more accurate story.

Does not account for fat distribution: Visceral fat (around organs) is far more dangerous than subcutaneous fat (under the skin). Two people with the same BMI can have vastly different health risks depending on where their body stores fat. Waist-to-hip ratio and waist circumference provide this critical context.

Developed from European populations: BMI thresholds were established primarily from European and white American data. Research shows that South Asian, East Asian, and other populations may develop metabolic complications at lower BMIs. Some countries use lower overweight thresholds (23 instead of 25) for these populations.

Ignores age, sex, and body composition: Muscle mass naturally decreases with age, and body fat distribution differs between men and women. BMI treats everyone the same regardless of these factors.

Not valid for everyone: BMI should not be used for children (use age-specific percentiles), pregnant individuals, very muscular athletes, or elderly adults with significant muscle loss. Use BMI as one data point among many, not as a standalone health assessment.
